Death Certificates

Procedure for establishing a death certificate in Subdivisional Councile

1) Death certificate of a man married to one woman who dies in a health center.

The following documents must be provided:

– Certificate of cause of death;

– Photocopy of the deceased’s national identity card;

– Photocopy of the marriage certificate;

– Photocopy of the national identity cards of two (02) witnesses, who may be siblings or adult children;

– Photocopy of the national identity card of the declarant, who is his wife;

 

2) Death certificate of a man married to several women who dies in a health center.

The following documents must be provided:

– Certificate of cause of death;

– Photocopy of the deceased’s national identity card;

– Photocopy of the marriage certificate of all wives;

– Photocopy of the national identity card of two (02) witnesses who are two of these wives;

– Photocopy of the national identity card of the declarant who is the head of the family;

3) Death certificate for an unmarried man who dies in a health center.

The following documents must be provided:

– Certificate of cause of death;

– Photocopy of the deceased’s national identity card;

– Photocopy of the national identity cards of two (02) witnesses, who may be his girlfriend, close relatives, or friends;

– Photocopy of the national identity card of the declarant, who is a close relative;

4) Death certificate for a man married to one woman who dies at home or in a health center.

The following documents must be provided:

– Death certificate signed by the district chief;

– Certificate of residence;

– Photocopy of the deceased’s national identity card;

– Photocopy of the marriage certificate;

– Photocopy of the national identity cards of two (02) witnesses, who may be his brothers and sisters or adult children;

– Photocopy of the national identity card of the declarant, who is his wife;

 

5) Death certificate of a man married to several women who dies at home.

The following documents must be provided:

– Death certificate signed by the chief  of the district;

– Certificate of residence;

– Photocopy of the deceased’s national identity card;

– Photocopy of the marriage certificate of all wives;

– Photocopy of the national identity card of two (02) witnesses who are two of these wives;

– Photocopy of the national identity card of the declarant who is the head of the family;

6) Death certificate of a person whose body has been discovered and who has their national identity card.

The following items must be provided:

– Law Enforcement Bodies report;

– In this case, the declarant is the head of the Law Enforcement Bodies report;
